Core Responsibilities
- Provide daily medication administration in the Medication Assisted Recovery program.
- Provide nursing assessments, medication administration, medication monitoring, and medication training to assist clients with learning how to self-administer medications, monitor response to medications on target symptoms, properly secure and store medications, and communicate concerns to providers; support referral and care coordination.
- Accurately document in the electronic health record daily on interventions, the client’s response, and the client’s progress in treatment.
- Care Coordination & Client Education - Coordinate holistic care for individuals receiving medication assisted recovery, including primary care, labs, mental health or substance use treatment, and other identified needs/care. Provide education on health and wellness, including, but not limited to, chronic health disease self-management, infectious diseases, and primary care.
- Team Collaboration & Compliance - Participate in a team-based approach to providing treatment, including collaboration with clinicians, nursing, healthcare providers, and other partners invested in the individual’s recovery. Attend clinical staffing(s) to collaborate with providers to coordinate treatment, including counselors, engagement staff, nursing, physician(s), and clinical leaders. Adhere to agency policies and procedures, accrediting body requirements for the Opioid Treatment Program (CARF), and federal and state guidelines (Illinois Division of Substance Use Prevention and Recovery, DEA, and SAMHSA).
- Engage in ongoing learning of current best practices in behavioral health treatment.
- Perform other duties as assigned consistent with this role.

- Clients receive safe, accurate, and timely medication administration and monitoring.
- Documentation in the electronic health record is complete, accurate, and entered daily.
- Care is well-coordinated across primary care, labs, and behavioral health services.
- The nursing team, providers, and program leadership view this position as a reliable, collaborative teammate.
- The program maintains compliance with CARF, DEA, SAMHSA, and state requirements.
Knowledge, Skills and Abilities
- Familiarity with addiction/recovery and behavioral health.
- Thorough understanding of trauma-informed care practices, essential for providing optimal support and services to individuals in a manner that is sensitive, empathetic, and effective.
- Ability to work as a team player.
- Ability to tolerate ambiguity, uncertainty, and change.
- Demonstrated above-average computer skills; proficient in Microsoft Word and Excel.
- Strong attention to detail and accuracy in medication handling and documentation.
- Ability to maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2.
Education and Experience
- Required: Minimum 21 years of age, an active, undisciplined State of Illinois LPN licensure, a valid driver’s license, reliable means of transportation, and proof of automobile insurance coverage. Ability to obtain and maintain current CPR certification.
- PREFERRED: Experience in substance use treatment, behavioral health, or medication assisted recovery settings. Experience with medication administration in an outpatient or Opioid Treatment Program setting.
