LPN Hospice | Inpatient
Faith Hospice has a rare opportunity for an LPN who feels the call to serve patients at the end of life and the opportunity to work as an essential part of an inpatient hospice care team. Our Trillium Woods Inpatient Hospice facility in Byron Center is seeking a compassionate LPN to join our care team.

At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides, and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling.
Responsibilities
- Assesses patient conditions and comfort, communicating changes to physicians, family, and caregivers
- Implements and revises individualized care plans, managing discomfort and providing symptom relief
- Administers medication, performs treatments, and coordinates with the Interdisciplinary Group for care planning
- Delivers specialized hospice training to staff, family, and caregivers for optimal patient care
- Educates patients and families on disease processes and provides emotional support
- Maintains communication with physicians and other disciplines
- Builds trusting relationships and completes required documentation promptly
Requirements
- LPN licensure in Michigan
- At least 2 years of med/surg, acute care, or skilled nursing experience
- Hospice and/or hospital experience preferred
- Passion for hospice nursing
