Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Deliver skilled nursing services according to prescribed orders.
- Assess signs and symptoms indicating physiologic and psychosocial changes.
- Collect, analyze, and interpret healthcare data and document nursing diagnoses.
Qualifications & Skills
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
Key Duties
- Provide compassionate, skilled, and compliant nursing care in patients' homes.
- Ensure safety and adherence to healthcare plans.
