About the role

Responsibilities

• Assess, plan, and implement patient care
• Educate patients and their families on disease processes, procedures, and medications
• Manage various patient needs such as wound care, tracheostomy, g-tubes, catheters, ventilators, and medication administration
• Create customized care plans to accommodate patient’s needs, preferences, and concerns, as well as special requests from their families

Requirements

• Bachelor’s degree in Nursing (BSN)
• Current California State License as a Licensed Practical Nurse (LPN)
• Current BLS certification
• Experience in home health care preferred

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Passion for providing high-quality care

Skills

• Effective problem-solving and decision-making skills
• Knowledge of medical terminology and patient care protocols
• Proficiency in electronic charting systems

Pay

$25 - $30 per hour
