LPN - Homecare

Seeking Licensed Practical Nurses (LPNs) to provide in‑home care for adult and pediatric patients in the Marshfield area.

Responsibilities

• Deliver skilled nursing care per physician orders and care plans.
• Assess, monitor, and document patients' physical and emotional health.
• Identify and communicate changes in patient conditions.
• Develop and implement personalized care interventions.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Maintain accurate clinical documentation.

Qualifications

• Current, unrestricted LPN license in Wisconsin.
• Current BLS/CPR certification.
• Current TB test or chest X‑ray.
• Preferred: at least 1 year of nursing experience.
• Strong clinical, communication, and documentation skills.
