About the role
This Licensed Practice Nurse (LPN) position involves providing exceptional home healthcare services under the supervision of a clinical manager.
Responsibilities
- Assess, plan, and implement individualized patient care based on specific needs and care plans.
- Educate patients and families on disease processes, procedures, and medication management.
- Manage various patient needs, including wound care, tracheostomy, G-tubes, catheters, ventilators, and medication administration.
- Create customized care plans addressing patient and family concerns.
Qualifications
- Valid LPN license.
- Ability to build long-term, one-on-one relationships with patients.
- Strong communication and clinical skills.
- Open to new graduates with training support, including shadowing and hands-on programs.
