About the role
We are seeking a Licensed Practical Nurse (LPN) to provide one-on-one homecare visits for patients requiring intermittent care.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care
- Initiate and revise care plans
- Administer medications and treatments
- Monitor patient responses
- Respond to changes in patient condition
- Support education and discharge planning
- Adhere to professional standards and policies
- Attend team meetings
- Participate in training
- Perform other assigned tasks
Qualifications
- Graduate of an accredited nursing program
- Current LPN license
- Valid BLS certification
- Current PPD or Chest X-Ray
- Prior LPN experience preferred but not required
- Complies with professional and legal standards
Skills
- Strong clinical assessment and intervention skills
- Ability to follow care protocols and documentation
- Excellent communication and team collaboration
