About the role

Our client is seeking a compassionate Home Health LPN to join their extraordinary care team in Pennsylvania. The ideal candidate will collaborate to provide patient care plans, support patients in the local area, and demonstrate proficiency with technology tools.

Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in and around Milford, PA
• Meet critical deadlines for charting
• Self-manage time and schedule efficiently
• Demonstrate experience with tablets, mobile phones, and EMR software

Schedule

Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year. Occasional travel may be required within the local area.

Requirements

• Associate degree or higher
• Valid Licensed Practical Nurse (LPN) certification/license in the state of Pennsylvania (PA)
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• ANY of the following valid licenses/certifications:
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Basic Life Support (AHA BLS)
• Valid Driver’s License, reliable mode of transportation, and auto insurance
