About the Role

We are seeking a compassionate Licensed Practical Nurse (LPN) to provide direct patient care in a home health setting. The role involves collaborating with healthcare team members, conducting patient visits per treatment plans, and ensuring compliance with regulations. You will observe and document patients' physical and mental conditions, report changes promptly, and educate patients and caregivers about care plans. Participation in case conferences, maintaining accurate documentation, and demonstrating sensitivity to diverse patient needs are essential.

Responsibilities

• Provide direct patient care in a home health setting.
• Collaborate with healthcare team members to implement treatment plans.
• Conduct patient visits and ensure compliance with regulations.
• Observe and document patients' physical and mental conditions.
• Report changes in patient condition promptly.
• Educate patients and caregivers about care plans.
• Participate in case conferences.
• Maintain accurate and timely documentation.
• Demonstrate sensitivity to diverse patient needs and adhere to clinical protocols.

Requirements

• Graduate of an accredited practical nursing program and licensed in the state.
• Current CPR/BLS certification, maintained throughout employment.
• Minimum of one year of nursing experience; home care or hospital experience preferred.
• Excellent clinical, communication, organizational, and time management skills.
• Valid driver’s license and auto insurance.
• Experience with EHR systems preferred.
