Role Overview We are seeking a compassionate Licensed Practical Nurse (LPN) to provide direct patient care in a home health setting. The role involves collaborating with healthcare team members, conducting patient visits per treatment plans, and ensuring compliance with regulations. The LPN will observe and document patients' physical and mental conditions, report changes promptly, and educate patients and caregivers about care plans. Participation in case conferences, maintaining accurate documentation, and demonstrating sensitivity to patient needs are essential responsibilities. Qualifications & Skills Graduate of an accredited practical nursing program and licensed in the state.Current CPR/BLS certification and maintained throughout employment.Minimum of one year of nursing experience; home care or hospital experience preferred.Excellent clinical, communication, organizational, and time management skills.Valid driver’s license, auto insurance, and experience with EHR systems preferred. Additional Details Mileage reimbursement offered.Day shift hours with no after-hours on-call.Must demonstrate sensitivity to diverse patient needs and adhere to clinical protocols.
