Role Overview Seeking a compassionate Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide direct patient care within a home healthcare setting. The role involves collaborating with the healthcare team, conducting patient visits, and ensuring compliance with treatment plans and regulations. Key Responsibilities Coordinate care with RN case managers and team members.Perform patient visits per physician’s treatment plan.Observe and document patients’ physical and mental conditions.Report changes to supervisors and physicians promptly.Educate patients and caregivers on care plans and health management.Demonstrate sensitivity to patients' needs and participate in team meetings and training.Complete accurate documentation of all care activities. Qualifications & Skills Graduate of an accredited practical nursing program and licensed in the state.Current CPR/BLS certification, maintained throughout employment.Minimum one year of nursing experience, with home care or hospital experience preferred.Strong clinical, communication, organizational, and time management skills.Valid driver’s license and auto insurance as per policy.Experience with electronic health records (preferred).
