About the Role

Seeking a compassionate Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide direct patient care within a home healthcare setting. The role involves collaborating with the healthcare team, conducting patient visits, and ensuring compliance with treatment plans and regulations.

Responsibilities

• Coordinate care with RN case managers and team members.
• Perform patient visits per physician’s treatment plan.
• Observe and document patients’ physical and mental conditions.
• Report changes to supervisors and physicians promptly.
• Educate patients and caregivers on care plans and health management.
• Demonstrate sensitivity to patients' needs and participate in team meetings and training.
• Complete accurate documentation of all care activities.

Requirements

• Graduate of an accredited practical nursing program and licensed in the state.
• Current CPR/BLS certification, maintained throughout employment.
• Minimum one year of nursing experience, with home care or hospital experience preferred.
• Strong clinical, communication, organizational, and time management skills.
• Valid driver’s license and auto insurance as per policy.
• Experience with electronic health records (preferred).
