LPN Home Health
Empath Home Health, a member of Empath Health, is seeking a caring full-time Home Health LPN to deliver individualized care in Collier County. About the Role
Home health visits in the Naples area (Collier County).
Schedule
Monday through Friday, 40 hours per week; weekend and holiday rotations required.
Responsibilities
- Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
- Observes patients and reports adverse reactions to medication or treatment to the Clinical Coordinator.
- Assists the Physician and/or Registered Nurse in carrying out the plan of care and performing specialized procedures.
- Performs patient and family instruction on diet, medication, and treatment.
- Collaborates with the interdisciplinary team to ensure seamless, holistic care.
Requirements
- Active Florida LPN license.
- Graduate of an approved nursing program.
- 1-2 years of LPN experience.
- Home health care experience preferred.
- Intravenous (IV) Certification strongly preferred.
