About the role

Spend more time caring for patients and less time charting with our AI-assisted documentation tools and the industry-leading WellSky EMR. These technologies help streamline charting, reduce after-hours paperwork, and allow you to focus on what matters most—providing skilled nursing care in patients’ homes.

Responsibilities

• Provide skilled nursing care in patients’ homes under the direction of the Registered Nurse and physician.
• Perform ordered treatments, medication management, wound care, and patient education within the LPN scope of practice.
• Observe, document, and report changes in patient condition to the supervising RN.
• Collaborate with physicians, nurses, therapists, and other members of the care team.
• Complete accurate and timely documentation while maintaining Medicare and agency standards.

Requirements

• Current Florida Licensed Practical Nurse (LPN) license.
• Valid driver’s license and reliable transportation.
• Excellent clinical, communication, and organizational skills.
• Home health experience preferred, but not required.
• Experience with WellSky or another home health EMR is a plus.
