Responsibilities

• Delivering high-quality clinical services within the scope of practice, following the care plan, and adhering to infection control procedures.
• Documenting patient visits accurately in electronic medical records and electronic visit verification systems.
• Participating in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Qualifications and Skills

• Current, unrestricted LPN license in the practice state.
• CPR certification, valid driver’s license, and access to reliable transportation.
• Ability to work flexible hours and serve diverse home environments.

Preferred Attributes

• Home care experience, strong organizational and communication skills, ability to manage multiple tasks independently.
