Responsibilities
- Displays a courteous attitude and respect for all residents, families, and staff.
- Acts as the bridge between Centralized Admissions Team and site clinical team ensuring a smooth transfer of patients.
- Maintains positive relationships with Centralized Admissions Team, supporting as liaison for site.
- PARTNERS WITH CENTRALIZED ADMISIONS TEAM ON MAINTAINING POSITIVE RELATIONSHIPS WITH CURRENT REFERRAL SOURCES.
- Anticipates, coordinates, and orders any equipment needed for a patient’s stay.
- Demonstrates proficiency in medical terminology.
- Supports scheduling medical appointments and transportation.
- Maintains electronic medical records for accurate census data.
- Assists with inventory and delivery of nursing supplies including assistance in managing medications, supplies, and equipment including ordering, receiving, storing and disposing of all items in accordance with policies, and procedures.
- BALANCES HEALTH UNIT COORDINATOR DUTIES WITH LPN DUTIES AND NEEDS FROM CLINICAL TEAM.
- PERFORMS OTHER DUTIES AS ASSIGNED.
Qualifications
- Current Minnesota LPN or RN Required.
- Prior experience as a Health Information Coordinator, Health Unit Coordinator (HUC) or Trained Medication Aide (TMA) required.
- MUST BE ABLE TO PASS MEDICAL TERMINOLOGY CLASS.
- Basic knowledge of computers, word processing, data processing, and copy machines.
- Strong leadership, written and verbal communication skills.
- Excellent organizational, time management and team-building skills.
- Self-starter with a desire to be a part of a team.
