About the Role

Responsible for providing overall leadership and management of the health and well-being of the residents within the community.

Responsibilities

  • Ensures that community follows all federal, state, and local laws and regulations as they pertain to clinical care.
  • Collects, analyzes, and reports occurrence trends, and develops or implements improvement plans as necessary.
  • Partners with Administrator and other team members to analyze and maintain all resident and team member health safety.
  • Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
  • Responsible for infection control programming in the community.
  • Oversees the safe and efficient distribution and administration of injections, nebulizers, and moderate wound care per state regulations.
  • Identifies ongoing needs and services of residents through the assessment and Service Plan process, including documentation for residents with a change in health care status.
  • Completes monthly updates and reporting to the responsible party.
  • Partners with other department coordinators to identify, review, and discuss potential changes in service or medication levels and updates the resident’s service plan as required by state regulations.
  • Participates in community awareness activities and community relations.
  • Maintains communications with residents' families and/or responsible parties regarding changes in care or health concerns.
  • Informs the Business Office of fee changes related to care needs.
  • Other duties as assigned.

Supervisory Responsibilities

  • Interviews, hires, and trains employees.
  • Plans, assigns, and directs work.
  • Appraises performance; rewards and disciplines employees.
  • Addresses complaints and resolves problems.

Requirements

  • Maintains current state license as a Licensed Professional Nurse or a Professional Registered Nurse per state regulations.
  • Experience in assisted living, home health, or long-term care industries.
  • Two (2) years' experience as a Licensed Nurse (LPN-RN).
  • One (1) year of nursing management experience.
  • Demonstrates competence in assessment skills, injections/medication administration, follow-up, and triage.

Skills

  • Language Ability: Ability to communicate effectively in the primary language of the residents. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Cognitive Demands: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Computer Skills: Proficiency in computer skills, Microsoft Office, and ability to learn new applications.

Work Environment

Works primarily indoors in a climate-controlled setting. Team members may be exposed to blood and/or body fluids with potential exposure to hazardous materials and infectious diseases. Possible exposure to unpleasant odors and chemicals as identified in the MSDS Manual.

Physical Requirements

  • Regularly required to stand and walk.
  • Frequently required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear.
  • Occasionally required to stoop, kneel, crouch, or crawl.
  • Must regularly lift and/or move up to 50 pounds independently and up to 100 pounds with assistance or mechanical devices.
  • Must be able to push up to 350 lb. (wheelchair).
  • Specific vision abilities required include close vision and distance vision.
