LPN Health Care Coordinator

About the role

At Summerhouse Ashton Manor, a thriving Discovery Senior Living community, we are seeking a Licensed Practical Nurse (LPN) to join our team. The ideal candidate will provide nursing care in accordance with the client's plan of care, including comprehensive health and psychosocial evaluations, monitoring of the client’s condition, health promotion, and prevention coordination of services.

Responsibilities

• Provide nursing care in accordance with the client's plan of care
• Conduct comprehensive health and psychosocial evaluations
• Maintain accurate records and documentation
• Monitor clients' conditions and adjust care plans as necessary
• Coordinate services with other healthcare professionals
• Ensure compliance with federal, state, and local laws and regulations

Requirements

• Two years LPN experience preferred
• Two years experience in assisted living or in a nursing home
• Licensed Practical Nurse license in good standing
• Ability to ensure community follows all relevant laws and regulations
• Good judgment, problem-solving, and decision-making skills
• Effective organization, time management, and communication skills
• The ability to handle multiple priorities and delegate tasks
• A dedication to seniors and their well-being
• The ability to work nights, weekends, and holidays
