About the Role
Axiom Gardens of Nashville is seeking a dedicated and compassionate LPN to join our healthcare team. As an integral member of our multidisciplinary team, you will provide high-quality patient care, support, and advocacy to our residents. You will work with a diverse population of residents, each with unique needs and stories, and collaborate with other professionals to provide comprehensive care. Responsibilities
- Provide direct patient care, including administration of medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Demonstrate excellent communication and interpersonal skills to ensure effective communication with residents, families, and healthcare professionals
- Participate in quality improvement initiatives and maintain accurate and up-to-date medical records
- Collaborate with the nursing team to ensure continuity of care and optimal outcomes
- Perform other duties as assigned by the Registered Nurse Manager or Director of Nursing

- Work environment: long-term care facility with a caring and supportive team atmosphere
- Work schedule: full-time or part-time positions available, with rotating shifts and on-call responsibilities
