Overview
Works under the supervision of the outpatient provider in care of the patients and their environment (including those needing special precautions) and performs specified treatments.
The individual must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served in assigned unit. The individual must also promote and maintain a safe work environment for staff as well as our customers, incorporating national patient safety initiatives.
What You Will Do
- Under the supervision of the outpatient provider, partners with patients and families to implement plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient.
- Incorporates evidence-based standards of practice.
- Collaborates with interdisciplinary team in the coordination of care.
- Facilitates safe patient transitions to the next care setting as part of plan of care.
- Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care.
- Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence based practice and quality measures.
- Demonstrates effective nursing skills (vitals, blood sugars, administering treatments and medications) in completing task accurately and efficiently.
- Follows protocols and physician orders.
- Demonstrates understanding of diverse patient care populations and ability to modify educational approach to ensure patient/family comprehension.
- Evaluates for patient comprehension and documents response.
- Demonstrates the ability to build trusting collaborative relationships with peers and multi-disciplinary team to support patient care needs.
- Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
- Demonstrates task prioritization appropriately to facilitate quality patient care and ensure patient care needs are met.
- Able to recognize and respond to emergency situations.
- Evaluates own needs for support and uses identified systems/resources to obtain assistance.
Qualifications
- Education: Program Graduate, Practical Nursing (Required)
- Experience: 1 year Acute care experience (Preferred)
- Licenses and Certifications: Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required), CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required)
