About the role

The LPN will follow agency policies and participate in related goals and plans, which promote inclusion as well as social and emotional growth for the individuals we support. The Licensed Practical Nurse is responsible for the health, well-being, and development of individuals we serve and works in conjunction with other staff members to provide the best possible care for all individuals. Supports the IMPACT mission, vision, and guiding principles.

Responsibilities

• Follows agency policies and participates in related goals and plans promoting inclusion and social/emotional growth.
• Provides care for the health, well-being, and development of individuals served.
• Works in conjunction with other staff members to ensure the best possible care for all individuals.
• Serves as a role model for the individuals we support, embodying the IMPACT mission, vision, and guiding principles.
• Supports individuals with trach care, as required.

Requirements

• Valid CT LPN License
• Valid CT Driver’s License
• 1-3 years on the job experience
• Experience working with individuals with ID/DD and physical disabilities exhibiting challenging medical, physical, and behavioral needs
• Willingness to work a flexible schedule, including different programs as needed

Qualifications

• CPR/First Aid, PMT, and DDS Medication Administration Certification, as required

Pay

$15.00 per hour

Schedule

Monday - Friday, 8:30 am - 2:30 pm (30 hours)

• Ability to work with individuals with developmental and intellectual disabilities
• Flexibility to work a variety of shifts and programs
• Strong communication and interpersonal skills
