About the role

Provide skilled nursing care and prescribed treatments in the home setting.
Care for adult and geriatric patients with a variety of healthcare needs.
Develop and implement patient-centered care plans.
Collaborate with physicians, caregivers, and interdisciplinary teams.

Requirements

• Current Delaware LPN license or Compact State license.
• Minimum 1 year of LPN experience.
• Home Health and/or Med-Surg experience preferred.

Pay

Hourly Pay Range: $25.52 - $38.28

Schedule

Full Time 40 hours a week
Day shift
One weekend/month
Two holidays/year
Two on-call shifts/month
