Field Nurse (LPN)

About the role

Provide holistic, patient-centered care to seniors and adults with complex needs in their homes, collaborating with an interdisciplinary team to develop and implement clinical plans.

Responsibilities

• Conduct independent home visits
• Support chronic conditions, wound care, phlebotomy, and medication review
• Engage hard-to-reach patients, ensuring continuity of care and improving patient outcomes
• Partner with care teams to deliver wrap-around services and support patients at home
• Contribute to value-based care models that enhance outcomes and reduce costs

Requirements

• Unencumbered LPN license in the state of employment
• Experience caring for adults and seniors with medical and social complexity
• CPR and BLS certifications
• Intermediate to advanced computer skills, including electronic health records and communication platforms
• Ability to travel independently with a vehicle meeting specific requirements
• Valid driver’s license for at least 5 years with a clean record
• Vaccination against COVID-19 (medical or religious exemptions considered)
