Field LPN - Home Care

About the role

BayCare HomeCare is seeking a skilled Home Health Field LPN to join our team in Dunedin, FL. This full-time position offers a flexible schedule with occasional weekends, allowing for a great work-life balance. You’ll provide hands-on care in patients' homes within the Dunedin area, helping them maintain independence and improve their quality of life.

Responsibilities

• Deliver patient care to assigned patients, including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse.
• Identify, implement, and maintain appropriate teaching plans for patients and caregivers under the direction of a Registered Nurse.
• Follow the plan of care and collaborate with the Registered Nurse regarding patient response to treatment and changes in status.
• Utilize nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Requirements

• Current LPN (Licensed Practical Nurse) license.
• Associate's degree or diploma in Nursing.
• 1 year of experience as a Licensed Practical Nurse.
• 1 year of home care experience.
• BLS (Basic Life Support) certification.
• Valid Florida driver’s license (required within 30 days of hire).
• Successful completion of Level II screening through the Florida Care Provider Background Screening Clearinghouse.

Schedule

• Full-time position.
• Shift hours: 8:00 a.m. - 5:00 p.m.
• Weekend work: Every 3rd weekend.
• On-call: Not required.

Facility: BayCare HomeCare – Dunedin, FL. Home visits will be conducted in the Dunedin, FL surrounding areas.
