LPN Direct Care - I/DD Residential

Responsibilities

  • Regularly check vital signs and report findings to an RN or physician.
  • Administer daily oral medications, injections, g-tube/j-tube feedings (as assigned), and provide treatments as prescribed.
  • Coordinate and attend medical appointments to ensure high-quality care.
  • Maintain accurate, up-to-date documentation in eVero and QuickMar including DA logs, MAR, nurse’s notes, encounters, and medical reports.
  • Aid in creating menus, cooking healthy meals, and managing portion control.
  • Complete wound care as needed, turn and position individuals, and check adaptive equipment.
  • Assist with ADLs such as personal hygiene, bathing, dressing, feeding, and nail care.
  • Attend Life Plan and clinical meetings.
  • Train DSPs in health-related tasks, medication administration, and safety protocols.
  • Maintain a safe, clean environment by following universal precautions.
  • Ensure the overall safety of individuals, colleagues, and self.
  • Become certified and maintain required trainings.
  • Assist individuals in achieving desired personal outcomes through advocacy.
  • Maintain LPN licensure.
  • Perform other duties as needed or assigned.

Requirements

  • Licensed Practical Nurse with 2-5 years of previous nursing experience with IDD individuals or hospital/nursing home experience.
  • Strong written and verbal communication skills.
  • Self-starter with excellent organizational and problem-solving skills.
  • Experience as a direct support professional is preferred.
  • Physical characteristics: capable of sitting or standing for long periods, moving throughout the workday, working in an open space floor plan, and occasional lifting up to 25 pounds.

Qualifications

  • Education: High school diploma or equivalent.
  • Experience: 2-5 years of previous nursing experience with IDD population or hospital nursing and/or nursing home experience.

  • Safe environment maintenance and advocacy.

$29.50 per hour

Schedule

Sunday-Thursday 8:00AM-4:00PM (40 hours)

Transportation

Provides transportation using an agency vehicle.
