Responsibilities
- Regularly check vital signs and report findings to an RN or physician.
- Administer daily oral medications, injections, g-tube/j-tube feedings (as assigned), and provide treatments as prescribed.
- Coordinate and attend medical appointments to ensure high-quality care.
- Maintain accurate, up-to-date documentation in eVero and QuickMar including DA logs, MAR, nurse’s notes, encounters, and medical reports.
- Aid in creating menus, cooking healthy meals, and managing portion control.
- Complete wound care as needed, turn and position individuals, and check adaptive equipment.
- Assist with ADLs such as personal hygiene, bathing, dressing, feeding, and nail care.
- Attend Life Plan and clinical meetings.
- Train DSPs in health-related tasks, medication administration, and safety protocols.
- Maintain a safe, clean environment by following universal precautions.
- Ensure the overall safety of individuals, colleagues, and self.
- Become certified and maintain required trainings.
- Assist individuals in achieving desired personal outcomes through advocacy.
- Maintain LPN licensure.
- Perform other duties as needed or assigned.
Requirements
- Licensed Practical Nurse with 2-5 years of previous nursing experience with IDD individuals or hospital/nursing home experience.
- Strong written and verbal communication skills.
- Self-starter with excellent organizational and problem-solving skills.
- Experience as a direct support professional is preferred.
- Physical characteristics: capable of sitting or standing for long periods, moving throughout the workday, working in an open space floor plan, and occasional lifting up to 25 pounds.
Qualifications
- Education: High school diploma or equivalent.
- Experience: 2-5 years of previous nursing experience with IDD population or hospital nursing and/or nursing home experience.

Schedule
Sunday-Thursday 8:00AM-4:00PM (40 hours)
Transportation
Provides transportation using an agency vehicle.
