LPN Crisis Care

About the role

Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County.

Responsibilities

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Cook up and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

Requirements

• Current license to practice as a Licensed Practical Nurse in the State of Florida
• Current Basic Life Support Certification
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus

Qualifications

• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license
