LPN- Hospice Crisis Care

About the role

JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team. This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Schedule & Territory

• Hours: Monday – Friday, (3) 12-hour shifts, 7:00 AM – 7:00 PM
• On-call: Every other weekend
• Territory: Crystal Lake, IL 60014

What You’ll Do

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
• Provide appropriate pain/symptom management; evaluate the patient’s response to treatments/medications

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience; community health, hospice, or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill

Pay

$26 – $38 hourly, based on years of experience.
