About the Role
CommWell Health is seeking a compassionate, high-quality care provider to join our team. You will partner with providers and care teams to ensure seamless, coordinated treatment, support patients and families in understanding health needs and next steps, document care accurately using our EHR system, participate in care huddles and team-based quality improvement initiatives, and serve as a key member of our Patient-Centered Health Home model.
Responsibilities
- Deliver compassionate, high-quality care to patients across the lifespan
- Partner with providers and care teams to ensure seamless, coordinated treatment
- Serve as a key member of our Patient-Centered Health Home model
- Document care accurately using our EHR system
- Participate in care huddles and team-based quality improvement initiatives
Requirements
- LPN / CMA License OR Certification, active and in good standing with the State of North Carolina
- Graduate of an accredited program
- Highschool graduated/GED
- Current CPR/BLS certification
- Valid Driver’s License
Preferred
- One (1) year of clinical experience in a primary care, community health, or outpatient setting
- Experience with underserved or rural populations
- Familiarity with team-based care or PCMH/PCHH models
- Bilingual

Qualifications
Preferably one (1) year of clinical experience in a primary care, community health, or outpatient setting. Familiarity with team-based care or PCMH/PCHH models is preferred. Bilingual candidates are encouraged to apply.

Strong communication skills, ability to work collaboratively, and proficiency in EHR systems are essential.
