About the role
The Client Relationship Specialist is a Licensed Practical Nurse (LPN) responsible for developing and maintaining client relations and intake of direct patient referrals.
Responsibilities
- Cultivate relationships with physicians and hospitals to facilitate the best Home Care solutions for their patients.
- Monitor HCR RightFax folders, label faxes, and attach electronic documents to patient records in Home Care Home Base (HCHB).
- Monitor the Intake Team email.
- Produce timely, accurate data entry.
- Enter orthopedic prescreens into HCHB according to agency protocol.
- Provide phone support to the department and triage calls to Intake staff.
- Deliver excellent customer service to internal and external customers.
- Participate in department and agency meetings.
- Perform other duties as assigned.
Requirements
- Graduate of an accredited practical nursing program and licensed to practice in NYS.
- Minimum of 4 years of experience in a clinical setting.
- Exceptional customer service and communication skills.
- Highly proficient and efficient working with a computer.
- Detail-oriented and able to work in a fast-paced environment.
Work Environment
The Client Relationship Specialist works primarily in an office setting and may occasionally be exposed to outdoor conditions. The role is classified as sedentary work, involving exerting up to 10 pounds of force occasionally and sitting most of the time.
