LPN Case Coordinator
About the Role
As a Licensed Practical Nurse (LPN), you will provide compassionate nursing care to our residents under the direction of a physician or Registered Nurse. Every day, you will assess resident needs and deliver essential physical or psychosocial care. Success in this role requires genuine compassion for patient well-being, an understanding of your critical role in ensuring their comfort, and the patience to explain procedures, answer questions, and maintain accurate documentation.
Responsibilities
- Utilizes the nursing process to assist in developing and implementing a comprehensive plan of care for the patient/family, promoting optimal patient outcomes.
- Participates in meeting the educational needs of co-workers and patient/family, utilizing principles of teaching/learning.
- Delegates nursing care based on knowledge of staff qualifications/competency and patient care requirements.
- Consistently demonstrates flexibility, accepts preceptor roles, teaches/learns new skills, and picks up extra shifts.
- Completes documentation according to policy and policy timelines.
Requirements
- Graduate of a National League for Nursing accredited school of nursing.
- Experience in public health or home health nursing.
- Active Licensed Practical Nurse (LPN) license in North Dakota.
- Basic Life Support (BLS) - CPR certification.
- Valid driver’s license and auto insurance.

Pay
$22.51 - $31.79 per hour.
