LPN Care Coordinator - GI Clinic- Full Time

About the role

We assist in data collection and the implementation of health care services. Our role involves supporting the health care team in providing high-quality personalized patient care. We engage patients to promote optimal health and manage acute exacerbations of chronic conditions. We apply the Nursing Process to ensure quality patient care.

Responsibilities

• Oversees the disease registry database.
• Conducts pre-visit chart reviews of patients.
• Collaborates with the health care team to coordinate care across the continuum.
• Participates in quality improvement initiatives.
• Performs other related duties as required.

Requirements

• Graduate of an accredited school of practical nursing.
• 2 years of ambulatory care experience (preferred 3 years).
• Current licensed practical nurse (LPN) license in state of practice.

• Effective verbal and written communication skills.
