About the role

Responsibilities
- Updates Individualized Treatment Plans (ITPs) when patients are receiving medication services only.
- Provides care management for patients with chronic health conditions affecting behavioral health conditions.
- Acts as a team player, listens to input from participants and other team members, and positively influences morale and workplace culture.
- Supports medication adherence and alerts prescribers to changes in physical health status, medications, behavioral health symptoms, and medication side effects.
- Takes vitals, draws blood, collects urine samples, and performs other general nursing duties.
- Ensures all medications are stored, tracked, and disposed of according to departmental policy.
- Utilizes EMR and video-conferencing software as required for patient care.
- Ensures compliance with federal laws regarding confidentiality, including HIPAA and 42 CFR, Part 2.
Requirements
- LPN with at least 3 years of experience in healthcare settings.
- Knowledge of chronic physical conditions.
- Behavioral health experience preferred.
- Valid Ohio professional license as appropriate.
- Experience working with individuals who have mental, physical, or substance use disorders.
- Punctual attendance and valid State of Ohio motor vehicle operator’s license.
- Proof of personal automobile liability insurance coverage upon request.
