- Hours – 24 hours a week.
- Pay Range – $30-32 per hour.

Responsibilities
- Affirms residents maintain admission criteria, monitors, and recommends alternative placement if necessary.
- Affirms residents have up-to-date care plans per state regulations.
- Maintains and updates resident charts and clinical progress notes.
- Communicates to physicians, staff, and family changes in resident’s needs or conditions.
- Maintains proper medication administration procedures and discusses any needed corrections, errors, etc., with staff.
- Communicates information in resident’s log that staff need to be aware of.
- Supervises medication practices and medical equipment being used in the building.
- Ensures staff clearly understand and follow all resident care procedures.
- Shares resident and family concerns with pertinent staff if appropriate.
- Works with manager to ensure emergency procedures are in place.
Qualifications
- Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations.
- A minimum of one year's experience in home health, assisted living, or long-term care is preferred.
- Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision.
- Exhibits a caring heart with a passion to care for the elderly.
