About the role
A Hospice LPN provides compassionate, end-of-life nursing care. This role focuses on comfort, symptom management, and emotional support for patients and families.
Responsibilities
- Deliver bedside nursing care, including medications, wound care, and symptom relief
- Monitor patient condition and report changes to the RN/Case Manager
- Provide education and emotional support to patients and families
- Document care accurately and follow hospice care plans
- Work collaboratively with the interdisciplinary hospice team
Requirements
- Current LPN license
- Strong communication and compassion
