Logansport Transition Specialist

Job Responsibilities

• Facilitate the implementation of a life skills curriculum, titled Next Steps.
• Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
• Evaluate the five “Transition Consultations” to be completed by every student under the guidance of an assigned teacher.
• Participate every week in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
• Do the follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months.
• Represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.
• Assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
• Assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
• Work with state, county and private agencies to provide re-entry services for youth transitioning back into the community.
• Assist students with obtaining social security cards, state identification, driver's licenses, birth certificates, and additional documentation.
• Assist students with transitioning back to public schools
• Assist students with employment, apprenticeship programs, and military service information

Qualifications

Requires a Bachelor’s degree in a related field and a minimum of two years of experience in a similar role, or an equivalent combination of education and experience. Must have excellent communication and interpersonal skills, as well as strong organizational and time management skills. Proficiency in Microsoft Office Suite is required. Experience working with youth and knowledge of Indiana's Department of Correction- Division of Youth Services policies and procedures are preferred.
