Responsibilities
- Delivering compassionate patient care
- Managing symptoms
- Collaborating with healthcare teams to develop and implement care plans
- Assessing and managing patient symptoms in hospice and palliative settings
- Providing patient and family education and support
- Collaborating with multidisciplinary teams to optimize care
- Documenting patient care activities accurately and timely
- Ensuring compliance with healthcare regulations and standards
Qualifications
- CA License
- Minimum of 3+ years' clinical experience
- ACHPN certification
- Current BLS certification
Preferred Skills
- Strong communication and interpersonal abilities
- Ability to work independently and as part of a team
- Knowledge of hospice and palliative care protocols
