Host Healthcare is seeking a local contract Physical Therapist for a local contract job in Peoria, Illinois.

Responsibilities

• Provide physical therapy services under the supervision of a licensed physician.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patients' functional abilities and develop appropriate exercise programs.
• Document patient progress and communicate with other healthcare providers as needed.

Requirements

• Current license to practice as a Physical Therapist in the state of Illinois.
• Experience in outpatient physical therapy preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Current CPR certification.
