Local contract position in Bourne, Massachusetts for a Home Health Physical Therapist.

About the role

Pride Health is actively hiring a Physical Therapist (PT) to join a growing Home Health team. This is a 14-week contract with a start date of 08/31/2026, offering 40 hours per week on an 8-hour day shift.

Responsibilities

  • Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments.
  • Develop individualized treatment plans to improve patients' functional mobility while meeting a productivity expectation of 30 points.
  • Deliver evidence-based therapy interventions and monitor/document patient progress accurately in Homecare Homebase (HCHB)/PointCare.
  • Collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards.

Requirements

  • Licensed Physical Therapist (PT) in Massachusetts.
  • Experience with HomeCare HomeBase (HCHB) for documentation and PointCare.
  • Prior home health experience.

Schedule

  • Monday - Friday, 8:00 AM to 4:30 PM.
  • No on-call or weekend requirements.
  • 36 guaranteed weekly hours.
  • Shift duration: 8 hours per day.

Pay

  • Local pay rate: $55.00 to $70.00/hour (W-2).
  • Mileage reimbursement between visits (from first to last patient).

  • Position 2 (Wilmington, MA - 01887): Dunstable, Tyngsborough, Dracut, Lowell, Westford, Chelmsford, Billerica, Carlisle, Acton, Concord, Bedford, Burlington, Tewksbury.
  • Position 3 (Needham Heights, MA - 02494): Boston and surrounding areas.
  • Position 4 (Westborough, MA - 01581): Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, Framingham area.
