About the role

This is a part-time daytime opportunity that offers a consistent schedule, supportive team environment, and the chance to make a real difference in a patient’s daily life.

Responsibilities

- Provide hands-on personal care and skilled support in the home setting - Assist with medication administration per care plan - Perform catheter care and general patient monitoring - Support G-tube feedings and medication administration as needed - Monitor for changes in condition and respond appropriately - Communicate with the patient’s care team (physician, NP, case manager) - Offer emotional support and reassurance to patients and families

Requirements

- Active LNA license in the State of New Hampshire (LPN or RN may also qualify) - Pediatric or home care experience preferred - Experience with G-tubes, seizure management, or complex care is a plus - Current CPR certification (training available through Interim HealthCare)

- Calm, compassionate, and dependable caregiver - Strong communication skills and ability to work independently
