LNA - Full-Time / Home Health Care NO NIGHTS OR WEEKENDS

About the Role

Join our small but mighty team at Taylor Community in Wolfeboro! This position offers a rewarding work-life balance with no nights and no weekends. As an LNA supporting our Independent Living residents, you'll provide personalized assistance based on each resident's individual needs and play an important role in our assessment and care planning process.

Responsibilities

• Assist residents with activities of daily living (ADLs)
• Provide individualized daily support based on resident needs
• Provide weekly personal shopping assistance
• Arrange and accompany residents to healthcare appointments
• Participate in assessments and care planning
• Communicate resident needs and changes with the interdisciplinary team
• Work collaboratively with other members of the care team
• Build meaningful relationships with residents while promoting independence and dignity

Requirements

• 1 year experience in long term care
• Valid NH Driver's license
• Must have reliable transportation on our campus
• NH Criminal Record and Pre-hire screening must be satisfied
