About the role

We are seeking a Licensed Independent Social Worker to provide mental health consultations in settings such as nursing homes and assisted living facilities.

Responsibilities

• Conduct comprehensive mental health assessments
• Develop and implement personalized treatment plans
• Offer individual and group therapy sessions
• Provide crisis intervention and management
• Collaborate with healthcare professionals to coordinate care
• Advocate for clients’ needs and rights
• Maintain confidential records in compliance with regulations
• Evaluate treatment outcomes and adjust plans as needed
• Educate clients and families on mental health topics
• Participate in ongoing professional education and training

Requirements

• Master’s degree in social work from an accredited institution
• Clinical licensure (LISW or equivalent)
• Specialized training in therapeutic modalities (e.g., CBT, DBT)
• Experience working with diverse populations
• Strong crisis management skills
• Effective communication and interpersonal skills
• Proficiency in electronic health records (EHR)
• Ability to work independently within a multidisciplinary team
• Strict adherence to confidentiality and HIPAA regulations
