About the role

We are seeking a compassionate Licensed Independent Social Worker to provide mental health consultations in various settings such as nursing homes and assisted living facilities.

Responsibilities

• Conducting comprehensive client assessments to identify mental health needs
• Developing and implementing personalized treatment plans
• Providing individual and group therapy sessions
• Offering crisis intervention and support
• Collaborating with healthcare professionals for coordinated care
• Advocating for client resources within the community
• Maintaining confidential client records
• Evaluating treatment outcomes and adjusting plans accordingly
• Educating clients and families about mental health issues
• Participating in continuing education to stay current with best practices

Qualifications and skills

• Master's degree in social work from an accredited institution
• Clinical licensure (LISW or equivalent)
• Specialized training in therapeutic modalities (e.g., CBT, DBT)
• Experience working with diverse populations
• Knowledge of community resources and services
• Strong crisis management skills
• Excellent communication skills and proficiency in EHR systems
• Minimum of 1 year post-graduate clinical experience preferred
• Ability to work independently and within a multidisciplinary team
• Commitment to confidentiality and adherence to HIPAA regulations
