Responsibilities
- Engage with patients across a wide range of backgrounds, needs, and levels of motivation.
- Develop clear, practical Lifestyle Medicine Action Plans that reflect the patient’s goals, readiness, and real-life constraints.
- Use motivational interviewing and applied psychological principles to help patients build confidence, address ambivalence, and take meaningful next steps.
- Identify root causes and recurring behavioral patterns contributing to health concerns, and efficiently summarize these insights in a way that is understandable and actionable for the patient.
- Determine the most appropriate next step in care, including independent resources, self-directed education, shared medical appointments, large group participation, and one-on-one visits.
- Deliver care across multiple formats, including one-on-one visits, small group discussions, and larger group sessions.
- Facilitate shared medical appointments, helping create a structured and supportive group environment.
- Support patients in building momentum, recognizing that LM care is often delivered in focused episodes rather than traditional ongoing follow-up.
- Coordinate care and communication with primary care providers, specialists, and related programs, including virtual care teams.
- Participate in student shadowing experiences, modeling effective patient communication, clinical reasoning, and efficiency.
- Engage in ongoing review and analysis of patient interactions, including deeper reflection and refinement of communication, decision-making, and facilitation skills to continuously improve performance.
Qualifications
- Demonstrates a broad, generalist foundation in primary care, with the ability to approach patients from a comprehensive, whole-person perspective.
- Licensed Nurse Practitioner or Physician Assistant (North Carolina licensure required; South Carolina licensure optional but must also maintain North Carolina licensure if applicable).
- DEA license.
- American Heart Association BLS certification.
- Expected to obtain board certification in Lifestyle Medicine within the first year of employment.
