About the role

Join team of 1 Physician Office Hours: Monday – Friday 8:00am – 5:00pm Embedded care team support (RN Care Managers, MSWs, CHWs, Diabetes Educator, PharmD) Lab, pharmacy & interpreter services Located less than 2 blocks from our main hospital, Atrium Health Carolinas Medical Center Access to additional System level care management resources and virtual technology Ideal Candidate Primary Care experience is preferred Passionate about caring for underserved, medically complex patients within a practice that embodies diversity of race, culture, ethnicity and language.

Pay

$47.50 - $71.25 based on qualifications, skills, relevant experience, and/or training
