Position Overview

The Life Skills Coach (LSC) is a community‑based role working directly with individuals who have sustained a brain injury or other neurological condition. The LSC provides functional supports on an individualized basis to help clients acquire, retain, and improve the skills necessary to live independently at home and access a variety of community‑based resources.

Responsibilities

  • Provide positive support and act as an advocate for the client.
  • Assist and guide clients in identifying choices and making informed decisions about activities and life.
  • Provide structure and support in household management, including maintaining cleanliness and establishing a safe, healthy, efficient routine.
  • Assist with participation in recreation and community activities to promote socialization and peer relationships.
  • Help improve community living and integration skills in settings such as shopping, church attendance, sporting events, club participation, classroom support, volunteer/employment support, etc., including providing transportation.
  • Teach clients how to participate in desired recreational and social activities within their community.
  • Provide training or assistance with mobility in the home environment, using adaptive aids, and accessing public transportation.
  • Assist with developing effective and appropriate communication and social skills.
  • Operate within organizational guidelines, policies, professional and ethical standards.
  • Maintain confidentiality of client and family information in accordance with HIPAA/company policies.
  • Treat clients, families, co‑workers, and outside parties with respect and dignity, honoring organizational values and ethical practices.
  • Provide functional supports to help clients acquire, retain, and improve independence based on established goals.
  • Help clients reintegrate and reconnect with their communities to encourage independence.
  • Identify barriers, triggers, or changes in functional status and develop proactive interventions.
  • Educate clients on problem‑solving skills and strategies.
  • Accompany clients to community locations and appointments to teach use of community resources and services.
  • Serve as a role model/mentor and maintain communication with clients, families, and referral sources.
  • Complete daily progress notes and required documentation, submitting them to supervisors and the payroll/invoicing portal.
  • Monitor and appropriately utilize billable hours with clients.
  • Conduct professional behavior at all times.
  • Participate in agency trainings and other necessary trainings.
  • Communicate regularly with organization personnel regarding issues or concerns.
  • Participate in team meetings as requested.
  • Perform other duties as assigned to meet program goals or as requested by the agency.

Qualifications

  • Experience with individuals with developmental disabilities and, in some cases, physical disabilities (preferred).
  • Reliable computer or device for daily documentation; proficiency with Microsoft Word, Excel, and Adobe Acrobat Reader.
  • BS/BA in a related field OR 1–2 years’ experience working with adults who are developmentally disabled or related experience (preferred).
  • Valid driver’s license.
  • Background check and driver’s record check.
  • Residence within a reasonable commute to the client.
  • Maintain required personal auto insurance with minimum coverage of $100,000/$300,000/$50,000 (bodily injury/bodily injury per accident/property damage) or $300,000 combined single limit.
  • Military background or experience with veterans and persons with brain injury (a plus).
  • Ability to lift 25/50/80 pounds regularly as required.
  • Strong written and verbal communication skills.
  • Strong interpersonal skills and ability to collaborate with team members.
