Life Skills Coach
Part-time position up to 12 hours per week with potential to increase hours based on Veteran needs.
About the Role
The primary focus of the Life Skills Coach is to help make life more rewarding for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries by providing high-quality home and community services. Life Skills Coaches support and carry out quality client care plans, ensure resources are engaged effectively to meet client needs, orient and educate clients and their families about health care plan services and procedures, support accurate documentation, and supply timely data for generating invoices. This role involves collaboration with Case Managers and family members to serve Veterans through dynamic social, recreational, wellness, educational, and individualized life skills training and activities in the home and community. The Life Skills Coach reports to their assigned Care Coordinator.
Responsibilities
- Travel to current or prospective clients’ homes
- Transport client(s) during client visits
- Communicate with health care professionals, clients, families, and the general public through verbal and written communication
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
- Implement patient care plans through home and community goal-related activities
- Work independently and manage one’s own time as well as the time of client(s)
- Attend community activities and outdoor activities
- Assist with organization planning, cognitive awareness, doctor's appointments, and household management
- Drive client(s) and lift up to 50 lbs
- Walk as part of client activities
Requirements
- Reliable transportation, valid driver’s license, and agency-required auto liability insurance (100k/300k Bodily Injury)
- Access to a private personal laptop or desktop computer for HIPAA compliance
