Part-time position up to 12 hours per week with potential to increase hours based on Veteran needs.
About the Role
The Life Skills Coach helps make life more rewarding for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries by delivering high-quality home and community services. Coaches support and carry out client care plans, ensure resources meet client needs, educate clients and families about health care plans, maintain accurate documentation, and provide timely data for invoicing. This role collaborates with Case Managers and family members to serve Veterans through social, recreational, wellness, educational, and individualized life skills training in home and community settings.
Responsibilities
- Travel to clients’ homes and transport clients during visits
- Communicate with healthcare professionals, clients, families, and the public
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
- Implement patient care plans through home and community goal-related activities
- Work independently and manage time effectively for both self and clients
- Attend community activities and outdoor activities
- Assist with organization planning, cognitive awareness, doctor's appointments, and household management
- Drive clients and lift up to 50 lbs
- Walk as part of client activities
Requirements
- High School Diploma (preferred)
- Experience with physical and cognitive impairments/disabilities (preferred)
- Experience with Traumatic Brain Injury (preferred)
- Experience with Post-Traumatic Stress Disorder (preferred)
- Veteran/Military experience (preferred)
- Reliable transportation, valid driver’s license, and agency-required auto liability insurance (100k/300k Bodily Injury)
- Access to a private personal laptop or desktop computer for HIPAA compliance
- CPR/AED/First Aid Certification (American Red Cross or American Heart Association only)
- Social Security Card
- Auto Insurance (100k/300k Bodily Injury)
