About the role

Auburn Homes and Services of Chaska, an Illuminus Community, offers assisted living, memory support, and short-term rehabilitation and long-term skilled nursing care to older adults. Auburn Homes and Services has provided care in Carver County since 1928 and strives to empower residents to live with purpose and passion.

Responsibilities

  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community, keeping in alignment with our mission, vision, and values.
  • Responsible for on-call services, as assigned.
  • Plans, organizes, and directs resident activity programs across the entire campus in compliance with regulations and resident desires. Ensures documentation of activities.
  • Leads, enhances, promotes participation, and updates activities of therapeutic value through collaborative planning, preparation, and evaluation.
  • Supports the selection and success of Activity staff through performance evaluations and related management responsibilities, including interviewing, coaching, and training.
  • Ensures all assessment, MDS, and care plan requirements are met in a timely manner while remaining HIPAA compliant.
  • Ensures interdisciplinary staff meetings are attended by staff with longevity.
  • Analyzes the psycho-social needs of individuals through interviewing, observation, family members, and other sources, and reports concerns.
  • Participates in meetings as requested, QA committee activities, and quarterly manager’s forums.
  • Effectively manages the financial position of the department within budgetary levels.
  • Performs other duties, as assigned.

Requirements

  • A Bachelor’s Degree in human services, social work, health care, or a related field.
  • Two to three years’ experience within a service industry supporting senior care needs.
  • Must be a qualified therapeutic recreation specialist or registered/certified activities professional.
  • Commitment to quality outcomes and services for all individuals.
  • Ability to relate well to all individuals.
  • Ability to maintain and protect the confidentiality of information.
  • Ability to exercise independent judgment and make sound decisions.
  • Ability to adapt to change.
  • Must have a valid driver’s license and a clean driving record, including auto insurance with minimum coverage requirements.
  • Imagination, creative ability, and an understanding approach to illness, spirit of cooperation, and pleasant personality.

Pay

$30.46–$37.79 per hour.
