JOB SUMMARY
ESSENTIAL JOB FUNCTIONS
- Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives.
- Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Will read and adhere to resident rights agreement and report any violations of same immediately
- Will maintain a calm composed attitude when dealing with residents, families, or staff who may exhibit challenging behaviors.
- Will work cooperatively with all disciplines including volunteers.
- Will provide life enrichment of therapeutic value, following the accessing, planning, implementing and evaluating process to include documentation of enrichment.
- Will promote optimal resident participation in activities and support transportation to/from activities.
- Will complete MDS documentation, care plans and assessments as assigned on a timely basis, maintaining confidentiality.
- Led resident council meetings, maintaining/sharing minutes through appropriate channels and resources.
- Will report any unusual resident’s behavior and health concerns to nursing staff.
- Maintains order and cleanliness in activity areas.
- Treats residents, family and staff with dignity and respect.
- Researches and supports supply fulfillment for life enrichment activities.
- Other duties as assigned
QUALIFICATIONS
- Commitment to quality programs and services for the residents/patients/clients.
- Excellent communications and human relation skills.
- Ability to relate well to the residents/patients/clients, family members / personal representatives.
- Ability to maintain and protect the confidentiality of information.
- Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.
- Ability to work independently with flexibility in scheduling activities.
- Ability to lead large and small group activities and get involved, i.e. wear costumes, use props, etc.
- Must have a valid driver’s license and a good driving record.
EXPERIENCE AND TRAINING
- High school graduate or equivalent
- Ability to utilize common word processing and document activities in medical record.
- One year experience with residents or individuals w/special needs.
