Licensed Vocational Nurse (LVN) – Primary Care

About the role

The position is located in various locations across Oakland (East and West) and MobileCare Van (Contra Costa and Alameda).

Responsibilities

  • Provide nursing care in ambulatory primary care clinics, including medication and vaccine administration, laboratory result monitoring, treatments and procedures, and patient education.
  • Assess patient needs through observation and communicate findings to the healthcare team.
  • Perform adult primary care procedures within LVN scope of practice, such as ear irrigations, suture removal, wound care, assistance with clinical procedures, and assist patients with limited mobility.
  • Administer and document medications while maintaining current pharmacology knowledge and competency.
  • Support patient flow, triage activities, care coordination, and provider workflows.
  • Maintain compliance with UCSF policies, infection control standards, regulatory requirements, and quality activities.
  • Deliver customer service through telephone, electronic, written, and in-person communication.
  • Identify patient education needs, provide education, and make appropriate referrals.
  • Respond to patients with empathy and coordinate care with the healthcare team.
  • Prepare patients for diagnostic and therapeutic procedures.
  • Maintain continuity of care through effective communication with patients, families, and healthcare team members.
  • Deliver direct patient care in Mobile Health Van, including immunizations, point-of-care testing, specimen collection, wound care, patient education, prepare and restock medical supplies, medications, vaccines, and emergency equipment, ensure vaccine cold-chain integrity and compliance with VFC requirements, coordinate logistics with drivers, Environmental Services (EVS), and clinic leadership, support community outreach events, document mobile patient encounters accurately in ApeX.
  • Provide clinical support at School-Based Health Center sites within LVN scope of practice, prepare clinical areas and maintain supply readiness, maintain infection control compliance, coordinate with school staff, clinic leadership, and operational partners, document SBHC patient encounters accurately in ApeX.

Qualifications

  • 1 - 2 years of experience of LVN
  • Basic computer skills with Microsoft Word, Excel, and Outlook
  • Experience using electronic medical record systems
  • Demonstrated ability to provide highly personalized patient care
  • Willingness and ability to work in mobile and school-based clinical environments, including variable schedules, outdoor conditions, and multiple community locations
  • Licensed Vocational Nurse in the State of California
  • Active American Heart Association CPR Certification

Preferred Qualifications

  • Proficiency with ApeX
  • Experience as a Licensed Vocational Nurse in a Primary Care/Family Practice ambulatory setting
  • Familiarity with the principles of a Patient-Centered Medical Home environment
  • Experience in mobile health clinics, school-based health centers, or community outreach programs
  • Experience working in FQHC or HRSA-regulated environments
