About the role

Hospice of the North Coast is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our Palliative Care team. The role includes skilled nursing care, symptom management, patient and caregiver education, and care coordination in the community.

Responsibilities

  • Provide skilled nursing visits for palliative care patients in their homes.
  • Assess patient needs and report changes in condition to the Registered Nurse.
  • Administer medications and treatments within LVN scope of practice.
  • Provide education to patients and caregivers regarding disease progression, symptom management, medications, and care techniques.
  • Respond to after-hours patient needs, triage calls, and urgent symptom management visits.
  • Collaborate with physicians, nurses, social workers, and the interdisciplinary team to ensure continuity of care.
  • Complete accurate and timely documentation in the electronic medical record.
  • Support quality improvement initiatives, chart audits, documentation review, intake activities, and clinical operations as assigned.

Requirements

  • Graduate of an accredited Vocational Nursing program.
  • Current California LVN license in good standing.
  • Minimum one year of LVN experience (hospice, home health, palliative care, or medical-surgical experience preferred).
  • Excellent assessment, communication, and critical thinking skills.
  • Compassionate, patient-focused approach to care.
  • Comfortable working independently while collaborating with a multidisciplinary team.
  • Valid California driver's license, reliable transportation, and current auto insurance.

Qualifications

  • Preferred Hospice and Palliative LVN Certification.
  • Experience with electronic medical records (EMR).
  • Previous hospice, palliative care, or home health experience.

Pay

  • $2.00 per hour shift differential for eligible hours worked.

Schedule

  • After-Hours / Monday - Friday 5:00pm to 10:00pm.
  • This is an on-site position based in Carlsbad.
