Responsibilities
- Facilitate hospital discharge process and patient assessment/engagement to program
- Performing medical assessment intake and discharge plan of care
- Maintaining current guest chart via existing CRM database daily
- Conducting medical/wellness checks, medication regimen education, preventative measures and medical insurance follow up
- Coordinating and providing linkages for guest’s medical appointments and follow-up
- Collaborate with Intake to ensure appropriate eligible to recuperative services and admission of required DMEs and Home Health needs
- Participating in weekly internal and external case conferencing, board review team meetings, monthly staff meetings, trainings, and in-services
- Providing advocacy on guest behalf when needed and in tandem with LVN manager instructions
- Adequately managing caseload of a minimum 15 guests with flexibility to manage more to meet the needs of the facility (i.e., vacancies, PTO etc.)
- Tracking and completing guest information on existing database specific to progress and plan of care
- Completing additional tasks/duties as required to meet the needs of the facility, not to impede primary responsibilities
- Following NHF policies and procedures
Requirements
- LVN License or bachelor’s degree in nursing
- Current American Heart Association BLS (CPR/First Aid/AED)
- Up-to-date TB s required
- Recent work experience with homeless patients (preferred)
- Bilingual in Spanish (preferred)
- Reliable transportation
- Finger printing/ live scan background required
- Proven ability to stay calm and focused when working under stressful situations
- Experience working with substance abuse and dually diagnosed guests, as well as patients from diverse socio-economic, ethnic, and cultural backgrounds
- Proven ability to meet challenging deadlines, prioritize and organize effectively
- Computer proficiency in MS Word, Excel, Outlook, web browser and proficient typing skills
Physical Requirements
- Frequent standing and walking
- Extended sitting
- Frequent bending/stooping, squatting, reaching above the shoulders, twisting, turning, kneeling
- Lifting/pulling/pushing/moving up to 50 pounds
- Utilizing conversational speech for effective verbal communication
- Noise level is high in some guest care areas
- Must be able to hear equipment alarms, overhead pages, and direct verbal communication
